familysupportcircleI downloaded the template and upon using the contact page, it shows up like this in live:

I don’t know how to fix it so it will show an actual real contact form
HashIt seems much like problem with the plugin so you need to contact the plugin developer. In the mean time, you can leave your website url here so that we can check for the issue.
You can even you contact form 7 plugin which works good with any theme.
